Complete application packet and return to coordinator

*Please be sure to include a check for $35 payable to Employers Reference Source to cover the cost of FBI/DMV screening
Individual Interview with Coordinator to discuss program and particular areas of interest/establish basis for match
Participate in 2 Training Sessions - Basic and Elementary or Adolescent specific based on mentor preference
Orientation by appropriate school staff members
Process of being matched with student
1. Referral by teacher, guidance counselor, social worker, parent or at GHS self referral
2. Discussion with student regarding participation
3. Telephone discussion with parent obtaining verbal permission
4. Formal letter to parent inviting student to participate with written permission slip to be signed
5. Initial match - observe 4 to 6 weeks
6. If initial match positive, continue throughout school year
Review meeting after first 6 to 8 weeks with other new mentors
Continued review meetings with other new mentors throughout first year of mentoring
Ongoing consultation with coordinator and school staff
