Application Process

Application Process:

Complete and submit our online application or a copy volunteer application  and mail it to our office.

Our coordinators will contact you to schedule an individual interview to discuss the program further.  In order to establish a basis for a match, we will discuss particular areas of interest and preferences as laid out in our matching form  which you can complete and submit here.   If you prefer, you may also print copies and bring them with you.
Printable matching form
Printable mentor preference form

We do a criminal background check and DCF search on all our volunteers. There is a cost to running this check and we ask that you make a $40 donation (check made out to Guilford Youth Mentoring) to cover the expense. (We are a non-profit 501(c)(3) organization that relies on financial support from our community.)  If this donation causes a financial hardship, Guilford Youth Mentoring will cover the background check fee and this will in no way hinder your becoming a mentor. Please complete the DCF authorization release form and return to our office at Guilford Youth Mentoring, 605 New England Road, Guilford, CT 06437.

You can print the corresponding mentor agreement and release form , complete and bring it along with cash or a check made out to Guilford Youth Mentoring to your interview.

Participate in a New Mentor Training Session.

Orientation by appropriate school staff members in school where match will be located.

Process of matching with student:
  • Referral by teacher, guidance counselor, social worker, parent or self- referral
  • Discussion with student regarding participation
  • Telephone discussion with parent obtaining verbal permission
  • Formal invitation to parent/guardian for child to participate with written permission slip to be signed and returned
  • If initial match positive, continue throughout the school year and possibly longer

Twice yearly review meeting with other mentors in each school, for the purpose of consultation, updates and information pertinent to mentors in each school.

Ongoing consultation with coordinator and school staff.

*A note about our method of communication...we generally use our Guilford Public School email accounts for daily correspondence.  We also use the service Constant Contact for important general program and community announcements.  Please be advised that these messages will come from the email address Guilfordmentoring@gmail.com
